Taking Control Self-Directed Home Care

Discover a new way to manage your home care—where you are firmly in charge. Taking Control Self-Directed Home Care provides programs empowering individuals with disabilities or chronic health conditions to shape their support to fit their lives.

Taking Control Self-Directed Home Care includes three programs:

  1. Consumer Directed Personal Assistance Program (CDPAP) Facilitator Services, which provides ongoing support and wraparound services.
  2. Veteran Directed Care, which allows disabled Veterans to have input into their choice of care providers and methods of care delivery.
  3. Extended In-Home Services for the Elderly (EISEP), which supplies consumer-directed in-home services within specified counties.

Facilitator Services by Taking Control Self-Directed Home Care

Discover a new approach to home care with Consumer Directed Personal Assistant Program (CDPAP)—designed for individuals with disabilities or chronic health conditions. This statewide Medicaid program empowers you to take charge of your care, giving you the freedom to choose who provides your assistance and how it’s delivered.

  • Hire the Assistant of Your Choice: Select, train, and supervise your own Personal Assistant.
  • Be Your Own Supervisor: Direct every aspect of your care to fit your lifestyle and preferences.

WNY Independent Living’s Taking Control Self-Directed Home Care Program is an approved CDPAP facilitator in partnership with Public Partnerships LLC (PPL), the statewide fiscal intermediary.

Facilitator Services include:

  • Continuous Support: Our facilitators assist with every step, from understanding program requirements to navigating the timekeeping system for tracking care hours.
  • Wraparound Services: Throughout Independent Living Centers, our facilitators can connect you to a wide array of services for people with disabilities.
  • Convenient Local Access: Facilitators are available at five Independent Living Centers across New York State serving the following counties: Erie, Clinton, Essex, Franklin, Jefferson, Lewis, St. Lawrence, Niagara, Genesse, Livingston, Monroe, Ontario, Orleans, Wyoming, Allegany, Cattaraugus, Chautauqua, Chenango, Delaware, Otsego, Schoharie, Tioga & Tompkins.

Veteran Directed Home Care

Discover a new way to receive home care services with Veteran Directed Care—designed to give disabled Veterans greater control over their support, their caregivers, and their daily lives.

  • Personalized Control: Decide who provides your care and how your needs are met, including hiring family or friends as personal assistants.
  • Flexible Support: Benefit from a monthly budget to purchase approved goods and services that help you live safely and comfortably at home.
  • Guided Assistance: Work with a dedicated Person-Centered Counselor to create a customized Spending Plan that reflects your goals and priorities.
  • Wraparound services from your Independent Living Center: including mental health resources, independent living skills training, and advocacy.

Eligibility Is Simple

  • Obtain a referral for Veteran Directed Care from the VA.
  • Have a phone for program registration and tracking your Personal Assistants’ work hours.
  • Be able to make decisions about your care—or appoint someone you trust to help manage your needs.

Extended In-Home Services for the Elderly

The Extended In-Home Services for the Elderly program, provided by your county or nation, provides an alternative way for seniors receiving home care services to have more control over who provides care and how it is provided. This program puts you in charge—so your care reflects your unique needs and preferences. Whether you wish to hire a trusted friend, a family member, or someone from your community, you remain at the center of every decision.

  • You select hire, train, and supervise the assistant of your choosing.
  • Gain flexibility and control over how, when, & by whom your care is provided.

How it works

Taking Control Self-Directed Home Care acts as the “Employer of Record,” handling all employment paperwork so you can focus on living your life. We provide:

  • Assistance with creating advertisements or finding potential assistants.
  • A comprehensive training manual to guide you in managing your services.
  • Support from Independent Living Center staff as you learn to supervise and direct your care.

Eligibility Is Simple

  • Obtain a referral from your Department of Senior Services
  • Have access to a telephone for program registration and tracking work hours.
  • Ability to make decisions about your care or designate a trusted individual to manage your services.

Serving Erie, Niagara, Genesee, Jefferson, St. Regis Nation and Seneca Nation.
