JOB TITLE: Healthy Living Coordinator

DEPARTMENT: Healthy Living Department

LOCATION: Buffalo, Niagara Falls, and Batavia, N.Y. (primary location flexible to one of these WNYIL offices)

DIRECT SUPERVISOR: Chief Operations Officer (COO)


Supervise, develop, and enhance the provision of Adult Behavioral Health Home and Community Based Services (HCBS), Community Oriented Recovery and Empowerment (CORE) Services, and services contracted through the Western New York Integrated Care Collaborative (WNYICC).


  1. Coordinate and maintain the day-to-day workflow for the provision of Adult Behavioral Health Home and Community Based Services (HCBS), Community Oriented Recovery and Empowerment (CORE) Services, and services contracted through the Western New York Integrated Care Collaborative (WNYICC).
  2. Map workflows, and draft and maintain procedural manuals.
  3. Implement Independent Living (IL) philosophy, policies and procedures.
  4. Communicate and work collaboratively with Western NY Independent Living (WNYIL) family of agencies to support coordinated service delivery.
  5. Ensure WNYIL family of Agencies is adhering to evidence-based models as well as guidance from the State Office of Mental Health (OMH) and Health Care Plans.
  6. Work with the COO to lead efforts to further enhance and develop the array of services funded through health care plans.
  7. Ensure all referrals are assigned within twenty-four (24) hours and followed up on within forty-eight (48) hours.
  8. Ensure eligibility of consumers for services.
  9. Monitor units of service and authorization dates in accordance with contracted Medicaid Managed Care reimbursement policies.
  10. Complete Billing Cycle Reviews as scheduled. Work with Accounting to reconcile claims.
  11. Ensure all Units of Service are accurately documented in WNYIL’s Electronic Consumer Record (ECR) within two (2) working days of the date of service.
  12. Ensure documentation is entered into WNYICC’s portal according to their processes.
  13. Review all service documentation to ensure quality and compliance.
  14. Assist in the hiring of appropriate qualified staff.
  15. Orient and train staff on needed skills to provide and document quality service.
  16. Verify staff maintain proper training and credentials to provide billable services.
  17. Coordinate with direct supervisors in each agency to provide direction and coach staff working with in the HCBS/CORE/WNYICC services.
  18. Remain knowledgeable in HCBS/CORE/WNYICC service models and the research that demonstrates their effectiveness.
  19. Complete training for WNYICC initiatives and act as a co-trainer for service delivery if necessary.
  20. Assist with measuring performance of sites and providers through tracking outcomes and developing periodic reports on site and staff performance.
  21. Contribute to the performance evaluations of staff who are providing HCBS/CORE/WNYICC services.
  22. Coordinate and facilitate team meetings with HCBS/CORE/WNYICC staff from the family of agencies when appropriate.
  23. Conduct outreach and engagement activities to increase referrals and community supports.
  24. Attend Agency’s in-service training and staff meetings as well as any other agency-related activities as required.
  25. Participate in maintaining a neat, clean and safe work environment.
  26. Perform other job-related duties as assigned by the supervisor.


  • Requires a minimum of a Bachelor of Arts (BA) preferably in Rehabilitation or a behavioral health field.
  • Must be a Qualified Professional in one of the following areas: Certified Rehabilitation Counselor (CRC), Licensed Mental Health Counselor, Licensed Masters Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Credentialed Alcoholism and Substance Abuse Counselor (CASAC) – Advanced Counselor.


  • Must have four (4) years’ experience in addressing issues of behavioral health.
  • Supervision experience preferred.


  • Must have a valid driver’s license and vehicle in order to travel to all WNYIL Agencies.
  • Ability to work independently at WNYIL Agencies and off-site visits to provide seamless coordination of services.
  • The ability to learn about Medicaid, Medicare, pooled trust, and other financial resources that may be a barrier or need to obtain services.
  • Must have excellent verbal and written communication skills, good organization, and analytical skills.
  • Must have intermediate level of knowledge of computers including the use of Excel spreadsheets, databases, word processing and networks.
  • Perform in a professional, conscientious, and efficient manner for the purpose of ensuring a better quality of life for persons with disabilities.


WORK ENVIRONMENT: Office/ Community

PHYSICAL DEMANDS OF THE JOB:  Sitting for extended periods of time.
Occasional lifting of not more than 15 pounds.

HOURS / SHIFTS: 37.5 – 40 hours per week. M – F 8:30 a.m. – 5:00 .pm.
Overtime rarely required.

CLASSIFICATION: Full-time, salary, non-exempt, Professional staff.
Range: $50,000 - $56,000
