JOB TITLE: Education & Outreach Training and Coaching Specialist

GENERAL JOB DESCRIPTION
Contact nursing homes and do presentations on Section Q and Open Doors. Outreach to nursing homes for the Good Neighbor Program (GNP) and assist the Transition Specialist for the GNP. Work with participants who are in their first sixty (60) days post transition and implementing a pre-set Person-Centered Plan (PCP) curriculum that will involve primarily face to face contact with participants in their homes.
SPECIFIC DUTIES & RESPONSIBILITIES
1. Complete training with the New York Association on Independent Living (NYAIL) Open Doors Training and Outreach Coordinator to become well versed in trainings to be provided.
2. Accept direction/feedback from NYAIL and Western New York Independent Living (WNYIL) administration on tasks to be completed and follow through with tasks.
3. Outreach, coordinate and schedule face to face visits with nursing home staff.
4. Provide in person trainings to nursing homes staff on Section Q of the Minimum Data Set (MDS), Open Doors and GNP with a pre-set power point presentation.
5. Received referral, outreach and coordinate home visit with participants in their first sixty (60) days post transition and implement/follow up on a pre-set PCP curriculum with those participants in their homes.
6. Work collaboratively with Transition Specialist, Peer Advocate and Housing Subsidy programs and provide them with GNP information.
7. Meet monthly Education & Outreach (E&O) and Coaching goal.
8. Assure accurate documentation of consumer services and required paperwork in WNYIL’s Electronic Consumer Record (ECR) database, as well as any additional program specific databases and assure that this task is done within two (2) business days of the date of service.
9. Manage/maintain education and outreach materials and maintain list of WNY Nursing Homes with up-to-date information.
10. Provide and manage an evaluation system to assess effectiveness of trainings and identify additional training needs.
11. Participate with the Open Doors staff in the development of trainings for on-site staff, annual trainings, NYAIL conference, regional trainings, webinars and on-on-one trainings.
12. Utilize database, WNYIL and funder documentation methods to collect information about trainings/outreach/coaching provided and their effectiveness.
13. Prepare reports on outreach and coaching provided and needed.
14. Attends Agency’s in-service training and unit meetings as well as any other agency-related activities as instructed.
15. Participate in maintaining a neat, clean and safe work environment.
16. Perform other job-related duties as assigned by the supervisor.
EDUCATION & TRAINING
• Bachelor’s Degree is required.
• Professional and life experience may be considered in lieu of, in full or part of academic credentials.
KNOWLEDGE & EXPERIENCE (if required)
- Must have required experience developing and providing trainings to groups.
- Experience with public speaking is required.
- Experience working in or with nursing home staff is preferred.
- Preferred candidates should have a working knowledge of issues faced by persons of all ages who are in need of long-term care services, particularly with those related to nursing home transition.
- Experience with Nursing Home Transition and Diversion (NHTD), Traumatic Brain Injury (TBI), Office for People with Developmental Disabilities (OPWDD) waivers as well as Medicaid Managed Long Term Care (MLTC) is preferred.
- Personal experience or direct experience in working with people with disabilities, understanding of disability-related issues preferred.
- Understanding of Independent Living Philosophy. Basic knowledge of the Americans with Disabilities Act and a desire to learn more about the public laws that uphold the civil rights of disabled individuals.
SKILLS & ABILITIES
- Ability to travel throughout the five (5) counties of Western NY (Erie, Niagara, Genesee, Orleans and Wyoming).
- Must have time management skills to maintain individual/community outreaches, workshop schedule, documentation and administrative requirements.
- Ability to work well independently as well as part of a team.
- Must have intermediate level of knowledge of computers including the use of Excel spreadsheets, databases, word processing and networks.
- Must have excellent verbal and written communication skills, good organization and analytical skills.
- Perform in a professional, conscientious, and efficient manner for the purpose of ensuring a better quality of life for persons with disabilities.
WORK ENVIRONMENT: Office / Community
PHYSICAL DEMANDS OF THE JOB: Sitting for extended periods of time.
Occasional lifting of not more than 15 pounds.
HOURS / SHIFTS: 37.5 – 40 hours per week.
M – F 8:30 a.m. – 5:00 p.m.
Overtime rarely required.
CLASSIFICATION: Full-time, salary, non-exempt, Professional staff.
Range: $42,000 - $45,000
